Registration forms are available for download below or can be picked up at the Hampden, Middlesex, Monroe, and Silver Spring Township Recreational Departments.

Registration reminders:
1. If registering with HMMS for the first time please provide a copy of your child’s birth certificate or passport.
2. A parent signature is required on the front and reverse side of the registration form.
3. ALL (recreational & travel) U09-U19 players – must provide a small photo stapled to the form.
4. ALL (recreational & travel) U09-U19 players – FULL first name and last name cursive signature on white label – NO NICKNAMES (if printing a form from the website please have your child sign their name on a piece of white paper and send with registration).
5. U15 & older players must pay the $5 surcharge which covers higher referee fees in the U15-U19 age groups.
6. NON Hampden, Middlesex, Monroe & Silver Spring residents must include the $10 non-resident fee with their payment.
7. Players trying out for travel or EFC must include the $10 non-refundable tryout fee.
