If registering with HMMS for the first time you must provide a copy of your child’s birth certificate or passport.
Please use the 2012-2013 Age Group Chart to determine the appropriate age group and registration form for your child

Registration reminders:
1. If registering with HMMS for the first time you must provide a copy of your child’s birth certificate or passport.
2. A parent signature is required on the front and reverse side of the registration form.
3. ALL (recreational & travel) U09-U19 players – must provide a small photo stapled to the form.
4. U15 & older players must pay the $15 surcharge which covers higher referee fees in the U15-U19 age groups.
5. NON Hampden, Middlesex, Monroe & Silver Spring residents must include the $10 non-resident fee with their payment.
6. Players trying out for travel or EFC must include the $10 non-refundable tryout fee.
Checks should be made payable to HMMS.
Registrations received after APRIL 10th will be considered late and the late registration fee will apply.
