MEDICAL FORM
- The form is to be completed for each player participating in the Tournament.
- You may substitute your local form if it contains the same type of info.
- The coach must submit these forms for verification at registration.
- If a player does not provide a medical release, she WILL NOT be permitted to participate in the Tournament.
