Guest Player Form
- This form is to be completed for all guest players participating in the Tournament.
- The coach must submit this form for verification at registration.
- If a player does not provide a medical release, she WILL NOT be permitted to participate in the Tournament.

- If you are unable to print this form, send an email to registrar@hmms-soccer.org and advise how you would like for us to send it to you..... (please include your registration number).
