APPLICATION INFORMATION

Use the application form to apply for the 2012 Hampden Memorial Day Tournament. 

NOTE:  Application will NOT
be considered complete until the payment is received. 
See below for payment instructions. 
Please do not submit an application until you have the payment ready to mail.

Detailed payment procedures will be provided when you click the "Save and Proceed to Checkout" button on application form. Additionally, the submission of this form, coupled with receipt of payment, certifies that you accept the following conditions:

"I understand the fee is not refundable after Team selections have been made (approximately April 15, 2012).  I understand that my Team must provide it’s own insurance and that in the case of an accident or injury while participating in or traveling to or from the tournament, HMMS Soccer is not liable for damages.  My team meets all the requirements for the Annual Hampden Memorial Day Soccer Tournament as outlined in the application information." 

 

Note:  After you click the "Save and Proceed to Checkout" button you will receive a confirmation page with the data you have provided and payment instructions ... we recommend you print a copy of this for reference. You may also receive an email (to the address entered) with this same information. Please click "Save and Proceed to Checkout" button only once -- if you see errors after submitting, email any changes to registrar@hmms-soccer.org) with your assigned Confirmation Number as the email Subject.

Your money must be received no later than two weeks after you have applied or you will automatically be placed on the Waiting List. Please do NOT send payment via any methodology that requires signature acknowledgement by the Treasurer as this requirement may delay delivery by 3 to 10 days.
