Any person interested in coaching or helping with a recreational, travel or Eagle FC team must complete and submit an HMMS Coach Registration Form.  In addition to the coach registration form, persons interested in coaching at the travel or Eagle FC level must also complete and submit a Travel Coach Application.

Please note: Pictures are only needed for persons interested in coaching a Travel or Eagle FC team.
